Job summary

Greenwich Home Treatment team have an exciting opportunity for a Band 6 Mental health Practitioner to join our well established team which has extended its hours ofoperation to provide a 24 hour service. The team provide a comprehensive service to meet the needs of the residents in the Borough of Greenwich. Come and join a successful and dynamic service based at Oxleas House , Greenwich providing an effective alternative to inpatient treatment for people in mental health crisis, 365 days a year.

The post is suitable for a Registered mental health Practitioner with recent experience in an acute adult inpatient, crisis or community mental health setting. You should enjoy working within a busy and challenging environment, and must be able to work effectively under pressure. You will have a sound knowledge of mental illness and therapeutic interventions and competence in mental health assessment. You will be creative and innovative in your approach, and will work collaboratively with patients and their carers in the formulation of care and crisis plans. Your communication skills will be well developed and you will be able to effectively liaise with other mental health teams, the local authority and emergency services.

Main duties of the job

The Home Treatment Team is an essential component of the Crisis Care Concordat 2014 and forms part of comprehensive borough-based crisis services, alongside the Mental Health Liaison, Inpatient care. The team is multi-disciplinary and employs a number of nursing, medical, Occupational Therapists and support staff who work closely to provide a viable alternative to hospital admission and a skilled resource to support early discharge. The team also gate-keeps all referrals for inpatient care, carries out assessments of people referred by GPs for urgent assessment, and Joint working with Community mental health teams.

  • To carry out effective biopsychosocial assessments of individuals in crisis referred to the team from community mental health teams, GPs and other sources.
  • To screen all referrals for home treatment as an alternative to in-patient admission.
  • To carry out effective risk assessment and management, seeking advice from multidisciplinary colleagues and other teams where needed.
  • To assess patients on the inpatient wards for early discharge and home treatment.
  • To provide a range of agreed clinical interventions to service users and their relatives at home aimed at increasing the individuals psychological stability.
  • To develop effective crisis plans with individuals which will improve their future resilience.
  • To provide phone advice to people in crisis as part of the Trusts Crisis Line
  • To assess the welfare of people discharged from inpatient wards within the past 48 hours, involving other teams and agencies as required within the parameters of the 48 Hour Follow-Up protocol.
  • To carry out assessments of mental capacity as required to do so.
  • To liaise collaboratively with hospital and community based l health and social care services, both within Oxleas and elsewhere, also with emergency services and nonstatutory bodies.
  • To maintain comprehensive and timely clinical records in accordance with Trust standards.
  • To work within a three shift system over seven days per week
