Job summary

Community Learning Disability Nurse, Band 6 Fixed Term - 12 months, 37.5 hours per week

We have an exciting opportunity to recruit a learning disability nurse to work as a Primary Care Community Nurse in Complex Physical Health. You will work in the Bexley Community Team for People with Learning Disabilities as part of the community LD nursing team, and 1/2 the time in the GP surgeries across Bexley, focusing on the needs of people with learning disabilities.

Main duties of the job

  • Work as part of a team of nurses and work closely with Bexley PCNs and GP practices
  • Work with PCNs and practice staff on improving health of patients with learning disabilities
  • Provide high quality health care to adults with learning disabilities to improve outcomes.
  • Work within a multi-disciplinary team (MDT) to enable people with learning disabilities to access support.
  • Show understanding of the complex needs of people with learning disabilities.
  • Support GP practices in reaching out to patients who haven't had an annual health check (AHC) and support patients to attend where necessary.
  • Work with PCN staff to implement best practices.
  • Work closely with local organisations to improve the uptake of AHCs for patients reluctant to attend appointments.

You will be supervised by a band 7 nurse within the team.
