Main duties of the job

We are recruiting for a Gynaecology Cancer Support Worker who shares our ambition and vision to deliver the Best of Care by the Best Peopleand has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff.

The successful post holder will give clinical and administrative support to co-ordinate the care for cancer patients throughout their treatment pathway and during the stratified pathway follow-up.

As a Cancer Support Worker you will ensure that patients' needs are assessed and responded to appropriately, signposting to local and national resources and support, using evidence-based patient information. Working alongside the Macmillan Gynaecology Clinical Nurse Specialists and the multidisciplinary team to provide support and advice for patients following a diagnosis of Gynaecological cancer, helping to manage the stratified follow-up pathway.

Working with the Macmillan Gynaecology Clinical Nurse Specialists and in partnership with the Macmillan Personalised Care and Support Facilitator to support all aspects of personalised care and support planning for your designated tumour site.

Work cooperatively as part of the MDT, assisting nursing and other clinical staff to deliver a high standard of holistic patient care

Attend weekly MDT meetings, assisting the CNS with MDT preparation, subsequent tasks, referrals and administration

Assist in provision of MDT contact details for patients and guidance about who to contact for what purpose

Assist CNS team to provide helpline access and triage calls using agreed protocols

Provide general telephone advice and information, signposting patients/family & carers to further support appropriately

Contribute to and assist patients and CNS team with Holistic Needs Assessments (HNA) and development of care plans
