The successful post holder will need to be a Registered Children's Nurse with experience and knowledge of the key attributes of nursing children with Complex Needs within the community setting.

As a Children's Community Senior Nurse, you will be expected to work as part of the multidisciplinary team to assess, plan, implement and evaluate nursing care. The post holder will work in partnership with the Paediatric Consultants and Tertiary centres to provide the best holistic care for the Babies, Children and Young People (BCYP) and their families / carers.

Main duties of the job

A resource within the Medway NHS Foundation Trust and other agencies across Medway and Swale who are involved in caring for this group of children.

Be responsible for the provision of competent community nursing support, to meet individual needs of the BCYP and their families/carers.

To encourage parental participation in the care of their child by providing specialist support and practical guidance to these families.

To provide a supportive and guiding role for families with a terminally ill child at home, participating in a 24-hour on-call rota, if necessary, to support symptom control management.

Promote and facilitate 'best practice' in all aspects of care delivered to the BCYP and their families / carers.

With all members of the Paediatric Nursing Team, establish a good working environment in which all BCYP and families receive a high standard of 'Family Centred Care' maintaining respect and privacy

Ensure that nursing documentation is complete, legible, accurate and relevant, therefore complying with the Nursing and Midwifery Council (NMC) and the Lord Laming Enquiry recommendations.

Refer on and have an in depth understanding of Safeguarding BCYP, Child in Need concerns ensuring that local and national guidelines including the London Child Protection procedures are adhered to.

To have a knowledge and understanding of incidents reporting and implementation of Paediatric Risk Management, using the DATIX procedures.
