We are recruiting for a Lead Pharmacist - Medication Safetywho shares our ambition and vision to deliver theBest of Care by the Best Peopleand has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff.
The post holder will promote the safe use of medicines across the organisation and be the Trust's main source of expertise in this area. As the identified Medication Safety Officer, they will service as the essential link between the identification and implementation of (local and national) medication safety initiatives and daily operations to improve patient safety with the use of medicines.
They will create new models, pathways and processes both internally and across traditional boundaries (professional and geographical) to enhance medication safety. They will also be a member of the pharmacy department's Senior Management team, invested in the development of the department and its people.
Main duties of the job
To be the Medication Safety Officer (MSO) for the Trust and professional lead for medicines safety across the organisation.
To lead the Trust's Medication Safety Group (MSG).
To collaboratively develop and deliver on a medicines safety strategy for the Trust.
To be an active member of the national medication safety officer network.
To advise and guide the Pharmacy Senior Management team on ways in which their services can support the medicines safety agenda.
To be a role model and provide support, mentoring and training, as needed, to members of the pharmacy team.
Lead the Medication Safety agenda and be responsible for the monitoring systems and provision of information to support a reduction in avoidable harm resulting from inappropriate medicines use across the Trust.

Promote the safe, rational and cost effective use of medicines in accordance with the Medicines Management Group recommendations, Medicines Efficiency project and other evidence based guidelines.
To evaluate the clinical appropriateness, safety and legality of prescriptions based upon the patients diseases, co-morbidities and other medication and treatments taken, contacting the prescriber to make agreed changes as appropriate.
Drug history taking and medicines reconciliation at admission and discharge
Promote efficient discharge planning inclusive of patient counselling and education.
Follow procedures to reduce medicine waste (e.g. by using patient own drugs)
Ensure efficient supply systems (e.g. one stop dispensing), reducing the risks of critical missed and delayed doses by working with the technicians/ATOs and dispensaries.
Work with the ward nurses, dispensary and pharmacy ward team to improve patient flow.
Attendance on consultant wards rounds, as appropriate.
Answer or refer medicine information queries as appropriate
