The post holder will:
Participate in the Learning from Deaths and Incident review processes, facilitating these and act as the first point of contact for all Learning from Death/Patient Safety related enquiries within the Trust
Be frequently exposed to emotional/distressing circumstances relating to the death or potentially avoidable harm to patients
Support the incident management process across the Trust
Be responsible for maintaining accurate data on the central monitoring database, ensuring that all relevant information is entered and accurate. This may contain complex, sensitive or contentious information.
Assist with the delivery and planning of the Trust-wide Structured Judgement Review (SJR) process, which forms part of the learning from Deaths policy, ensuring that reviews are undertaken in line with national and Trust priorities. This includes involvement in continuous improvement, data collection, analysis, report preparation, interpretation and presentation of information, and contributing to the provision of timely mortality information
Assist with the delivery and planning of the Trust-wide Incident Review process, ensuring that reviews are undertaken in line with national and Trust priorities. This includes involvement in continuous improvement, data collection, analysis, report preparation, interpretation and presentation of information, and contributing to the provision of timely mortality information
Be responsible for ensuring relevant case notes are available for different areas to review as appropriate
Be responsible for ensuring that Structured Judgement Review forms/72 hour report forms are completed for each case reviewed and that these are entered onto the central/National monitoring databases where appropriate to facilitate identification of key themes and trends
Facilitate with the dissemination of learning from reviews throughout the Trust
Be responsible for the planning of, and administrative support for the specialty mortality and morbidity meetings, liaising with the specialty mortality leads to ensure that relevant cases are reviewed and discussed in this forum and provide administrative support, including formal minute-taking where required
Working with the Patient Safety Improvement Leads, support healthcare professionals, and disseminate and communicate statistical information
Produce and report information (both verbally and in writing) using both the local databases and external mortality indicator provider platforms including NHS Digital and Dr Foster and NRLS (later LFPSE), adjusting presentation styles to meet the requirements of different audiences as required, including providing monthly written reports for Care Groups.
Undertake investigations of areas of concern as identified through the mortality/incident review process
Work with clinical trainers to organise and support the Trusts Structured Judgement Review training programme
Support a positive learning environment and culture where staff are encouraged to use patient safety data as a tool for development through delivery and development of training packages to staff across the organisation
Support the development and implementation of Learning from Deaths and Patient Safety policies and standard operating procedures across the organisation including making recommended changes as a result of lessons identified and/or national best practise and regulatory changes
Support the implementation of National Guidance, integrating with the various services across the Trust
Deputise for the Learning from Deaths Lead/Patient Safety Practitioner where necessary
Supporting the effective and appropriate use of trust resources, including ordering and maintaining office equipment and supplies
