Main duties of the job
The successful postholder will provide a specialist frailty pharmacy service to the Emerald Frailty Unit within the Trust whilst working alongside other pharmacists, technicians and assistants as part of an enhanced and extended clinical service.
This post will involve leading on medicines optimisation strategies including palliative and end of life care. To utilise tools such as STOPP-START, STOMP, 7 STEPs in response to the National Overprescribing Review pledge of September 2021 around polypharmacy and overprescribing.

There will be specialist Rapid Assessment Care of Elderly (R.A.C.E) clinics throughout the week and daily ward rounds that the post-holder will be able to support with alongside the Frailty specialist pharmacy team.
The post holder will gain experience in taking on additional areas of responsibility including: specialist multi-disciplinary ward rounds, governance and risk, research and audit, and antimicrobials. The post holder will liaise between wards, departments and pharmacy to ensure that pharmacy services are tailored to meet the Trusts and patients need
You will work as part of the frailty team providing an enhanced and extended clinical service to support Medicines Optimisation in an exciting new Frailty Pathway.
To provide and develop a comprehensive, patient-focused, frailty clinical pharmacy service promoting the safe, appropriate and clinically effective use of drug therapy.
Provide patient focused pharmaceutical care to patients, to achieve definite outcomes that improve patients quality of life.
To maximise patient care on defined areas within the Trust by planning, managing, monitoring, advising and reviewing specialist pharmaceutical care programmes for patients.
To contribute in multidisciplinary frailty medicines optimisation ward rounds and additional shared-decision patient consultations for specialist structured medication reviews.
To contribute to R.A.C.E clinic assessments of frailty patients including Comprehensive Geriatric Assessments (CGAs) as required.
Evaluate the clinical appropriateness, safety and legality of prescriptions based upon the patients diseases, co-morbidities, other medication and treatments and liaise with the prescriber to make agreed changes as appropriate.
Ensure efficient supply systems (e.g. one stop dispensing) reducing the risks of critical missed and delayed doses through working with medicines management pharmacy technicians and assistants and dispensary staff.
Promote efficient discharge planning inclusive of patient counselling and education.
To promote safe transfer of care around medicines to various primary care settings across the interface. This would include close liasons with interface, Primary Care Network and Medway & Swale Medicines Optimisation teams.
Work with the ward nurses and other members of the multi-disciplinary team, and pharmacy colleagues to improve patient flow.
Promote the safe, rational and cost effective use of medicines in accordance with the Drug & Therapeutics Committees recommendations and other evidence based guidelines.
Participate in adverse drug reaction reporting, clinical contribution monitoring and clinical risk management
Follow procedures to reduce medicines wastage (e.g. by using patients own drugs)
Attendance on consultant wards and board rounds
Answer or refer medicines information queries as appropriate.
Assist in drawing up prescribing guidelines and protocols (e.g. IV monographs, formulary guidelines, shared care guidelines).
Liaise with other pharmacists, doctors and GPs to ensure efficient transfer of clinical and medicines management issues.
Liaise with other health care professionals to design, implement and monitor therapeutic plans.
To promote high quality, cost-effective and safe prescribing.
To participate in the training and development of other pharmacists and technicians within pharmacy.
To assist in the provision and development of clinical pharmacy services across the Trust
