We are recruiting for an Interface Frailty Medicines Optimisation Pharmacist as part of our next expanding Integrated Medicines Optimisation Pharmacy Team (IMOPT). Main duties of the job

The pharmacy department sits within the Unplanned and Integrated Care Directorate and provides medicines optimisation services to the entire Trust. The Frailty Directorate is undergoing an exciting expansion with the development of a new Frailty 7-day service , Rapid Access Clinic for Elderly (R.A.C.E) outpatient clinics. This role would be supporting the development of a new exciting expansion within the department to create an Integrated Frailty Medicines Optimisation Pharmacy Network and a desire by the team to focus on continuous improvement of our services for the benefit of our patients. Education and Training to ensure development of our staff is a key priority.

This role will be cross sector working in the community with multiple agencies and Primary Care Networks (PCNs) to support with complex medication reviews for patients in their own homes or in care homes. The role will involve managing the delivery and development of the clinical pharmacy support and input of multidisciplinary reviews for complex, vulnerable, frail & elderly patients across Kent, Medway and Swale in the community. The role presents an exciting opportunity to work as part of an interagency, multidisciplinary team to support patients living with frailty in the local community. This is part of an expanding Frailty network and Integrated Medicines Optimisation Pharmacy Team (IMOPT) supporting the domiciliary care of vulnerable patients alongside medicines management and governance in rehab units.

This post will involve leading on medicines optimisation strategies including palliative and end of life care. To utilise tools such as STOPP-START, STOMP, 7 STEPs in response to the National Overprescribing Review pledge of September 2021 around polypharmacy and overprescribing; reducing medication waste.

Qualifications

Essential

  • Master's degree in Pharmacy
  • Registered with the General Pharmaceutical Council as a Pharmacist in the UK (or eligible to be)
  • Specialist post-graduate qualification such as clinical diploma or equivalent experience

Desirable

  • Independent prescribing qualification

Knowledge

Essential

  • Demonstrates knowledge of medicine in frailty and elderly care
  • Understanding of frailty syndromes and principles of medicines optimisation
  • Good core therapeutic knowledge
  • Knowledge of and ability to apply all aspects of regulatory legislation and guidelines

Experience

Essential

  • Hospital pharmacy experience
  • Understanding of working effectively within multidisciplinary groups
  • Evidence of good ward practice, medicines management and participation on ward rounds
  • Proven experience of delivering responsibilities and objectives to time in previous posts

  • Demonstrates ability to identify problems, analyse root cause and propose solutions for complex problems
