Main duties of the job
The successful postholder will work alongside the PCN Pharmacists to support integrated medicines management
As a Pharmacy Technician you will develop and deliver a comprehensive medicines optimisation hub within the PCN setting to ensure a safe and efficient service is maintained. This will include Medicines Reconciliation, review of discharge medication, patient counselling and prescription management
In addition you will support the Clinical Pharmacist in Structure Medication Reviews (SMR) and the training of PCN & practice based staff.

Job responsibilities
To work alongside the PCN Pharmacist to support medicines management and to escalate issues outside limitations.
Develop and deliver a comprehensive medicines optimisation hub within the PCN setting to ensure a safe and efficient service is maintained, delivering a range of medicines management activities to include review of discharge prescriptions and outpatient letters to ensure patients medicines are reconciled, patients are counselled and working with the practice team, ensure repeat prescription lists are updated and reviewed accordingly.
To also include booking follow up bloods and escalate any issues that cannot be resolved as appropriate, with particular focus of high risk medications.
Support the reduction in: readmission for preventable medication related events; adverse events; and wastage of medication
Contribute to the efficient and effective delivery of medicines to patients in care homes. Resolve prescription queries from recent discharge prescriptions
Support the Clinical Pharmacist in Structure Medication Reviews (SMR) i.e. organise necessary monitoring tests prior to SMR. Support and review medication with the PCN Pharmacists and facilitate recommendations for prescribing changes to improve outcome and ensure prescribing of the most cost-effective product.
Support the clinical and administrative colleagues to process a wide range of medication related tasks. Implement efficient ordering and return processes to reduce medication wastage while promoting safe use of medication, reporting of medicines related incidents and pro-actively preventing safeguarding incidents.
Provide expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.
Pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on the PCN.

- L3 QCF Diploma in Pharmaceutical Science or GPhC accredited equivalent or GPhC equivalents prior to July 2011
- Registered as a Pharmacy Technician with the General Pharmaceutical Council
