Main duties of the job
You will be required to lead the pharmacy provision of the trust-wide clozapine services.
Work collaboratively with the lead clozapine nurse to ensure a holistic wrap-around care service for all clozapine patients.
To provide relevant and appropriate information and advice for patients and staff considering clozapine, about the drug and the implications for its use.
To manage overall the entry of the patient into the clozapine monitoring service, including the attendance and input into any multi-disciplinary team (MDT) meeting to advise on initiation processes for clozapine.
To be part of any community titration MDT advising on the processes and highlighting the roles and responsibilities of other professionals within that initiation period.
To support the appropriate increased uptake of clozapine use across the trust.
To ensure registration for each patient with the clozapine monitoring service. This involves organising the initial blood test, arranging or overseeing the clozapine titration on the ward or as an out-patient, and advising staff involved in the care of the patient.
