Job summary
The NHS Long Term Plan (LTP) commitment for tobacco dependency is to support people to quit smoking. The LTP aim is that all patients admitted to hospital who are smokers will be offered NHS funded tobacco treatment services, as well as some high-risk outpatient groups.
Lincolnshire Partnership Foundation Trust, working in partnership with Lincolnshire Community Health Service, is committed to encouraging and supporting smokers in our services to quit. We recognise that tobacco dependency is a chronic, relapsing clinical condition that prematurely kills at least half of people who smoke. Smoking is seen as a medical condition that can be treated, rather than as a lifestyle choice. This role will include working in Mental Health and Community inpatient wards to offer and provide high-quality support for treating tobacco dependency.
We are looking for a motivated, enthusiastic individual who can work as an effective team member, demonstrate good personal communication skills and actively promote excellent team and interdisciplinary relationships.

Main duties of the job
To work with the tobacco dependency treatment service lead to provide behavioural support and treatment in relation to tobacco smoking.
To support the day-to-day running of the service to ensure safe, well-organised and evidence-based care is offered routinely to all identified smokers and provided to all that access the service. Provide support in a range of settings as required for the delivery of smoking cessation service.
To support the delivery of the service, key performance and reporting indicators for tobacco dependency.
Organise own workload and work unsupervised in a busy team.
Maintain a high standard of work and prioritise your own workload according to the needs of the service and individual needs, including equality, diversity and social inclusion in all aspects of work.
Take referrals into the service and work with patients to identify the most appropriate tobacco dependency treatment pathway.
Identify and engage with individuals and groups from target areas.
Establish and maintain good relationships with patients accessing Mental Health or community hospital Services and members of staff within these settings, ensuring a high degree of confidentiality and complying with Information Governance policies at all times.
Support patients to make informed choices about their health and wellbeing and support them in initiating and sustaining appropriate behavioural changes to improve their health.
Record and report activities and results.
