Job summary
We have an exciting opportunity to join our Older Adult (OA) CMHT at Witham Court as a full time Band 5 Mental Health & Wellbeing Practitioner. This is a new role within the team covering the Lincoln & Gainsborough area combining clinic and home visits.
We are seeking to recruit a dedicated, enthusiastic and dynamic practitioner with a keen interest in developing existing skills with OA experiencing serious mental illness.

Clinical Supervision will be provided by a Clinical Psychologist in conjunction with the Team Co-ordinators.
The team supports flexible working with a combination of office and home based working.
The Team carries a caseload on the Mental Health and Dementia Pathways and is supported by Consultant Psychiatrists, Speciality Medics, ACP and nursing staff.
The post will entail supporting our existing patients on the Mental Health Pathway requiring specialist secondary services following the Divisional pathways and protocols.
We hold a weekly MDT meeting and aim to provide timely diagnosis, support and decision making to facilitate optimal management and life outcomes for those in need.
Main duties of the job
The post holder will be expected to work alongside patients, families, carers and MDT's to co-ordinate care, supporting collaborative decision-making in relation to care and treatment. This will include the fomulation of risk assessments and management plans.
The post holder will deliver a set of psychologically informed interventions, aligned to cognitive-behavioural principles, based in the best evidence, to address problems often experienced by people with severe mental health problems. This will require engagement with families and carers to develop care plans focused on strengths, and which are outcome based.
To work under close case supervision from a suitably qualified professional to ensure the delivery of high quality collabarative care planning as a valued member of the MDT.
To work under close clinical skills supervision from an HCPC registered Clinical Psychologist accredited to provide specified psychologically informed - wellbeing-focused interventions including:
Behavioural activation and graded exposure using the GOALS programme
Improving sleep
Building confidence
Teaching problem-solving skills
Recognising and managing emotions
Guided self-help for bulimia and binge-eating
Support with medicines management
You will sit within a significantly growing countywide psychology department consisting of Consultant Clinical Psychologist, 2 x MHWPs, 3 x Psychological Interventions Facilitators and 2 x Clinical Psychologists (starting in post in Oct 25).
