Job summary
An exciting opportunity has arisen for a part-time (15 hours per week) Community Mental Health Support Worker to join Louth Community Mental Health Team (CMHT).
We are looking for a Community Mental Health Support Worker with demonstrable skills, enthusiasm and compassion to work with service users who have a range of complex mental health needs who is focussed on supporting service users to recover.
Louth CMHT provide planned and effective mental health treatment alongside the service user, their carers and other services in line with the goals of the service user. The CMHT ensures service users needs are met by using mental health support services across the breadth of the community. You will be a key person to ensure this is delivered by working collaboratively to achieve the best outcomes for service users
The Community Mental Health Support Worker will contribute to the ongoing assessment, planning, implementation, and evaluation of care provided to service users. The Multi-disciplinary Team holds overall responsibility for the care provided with each service user, with you being the identified keyworker for a cohort of service users in the team who is the point of contact for them and their carer.
As part of the role, you will be supported to grow and develop in a manner and pace that you wish to. Regular management supervision will be provided and opportunities for continuing professional development within LPFT.
Main duties of the job
To provide a range of patients with mental health support in the community, implementing plans of care, utilising personal autonomy and utilising the support of the Multi-disciplinary Team (MDT) when needed, to support individuals with their ongoing recovery.
To work with a range of agencies within the CMHT and externally, to sign-post patients to appropriate sources of support or provide short, moderate or longer-term interventions to support their recovery, based on their clinical need.
The post will require working with the team to support the running of the service, managing a caseload, attending meetings, and taking on additional responsibilities as and when required. The ability to be flexible and organised is essential.
