Job summary

An exciting opportunity has arisen for a Carer Champion on Ellis ward for 37.5 hours per week.

We are looking for a highly motivated and enthusiastic, self-motivated individual to join our team of Carer Champions. The successful applicant will work closely to support family and carers whose loved one has been admitted to Ellis Ward. The role will support the team on Ellis Ward to ensure that relatives and carers are fully involved in patient care, ensuring relatives are part of the decision-making process in patient pathway, and working in conjunction with the team on Ellis Ward to signpost carers and relatives to appropriate support and information.

We are looking for someone who can understand and relate to the needs of family and carers using LPFT inpatient services and who is willing to work flexibly to attend carer support and education groups alongside carers.

Main duties of the job

This is an exciting new development within LPFT and recognises the important role carers play in our services and the need to demonstrate proactive support, especially at these times. The purpose of the role is to engage with our carers, meeting their needs and ensuring that the Division support for carers is personalised with a consistent approach across the pathways.

Your role within the division will be to support the Quality Lead, Matron and Service Manager for Carers on the development, implementation and delivery of carer outcomes within the services. You will be responsible for providing information resources, attending LPFT carer support groups. You will also lead on the development of planned visiting times within acute inpatients during this Covid-19 pandemic and ensure that carers feel supported and listened to.

You will support the Matron and Quality Lead in running regular carer involvement and consultation events and working with carer ambassadors within teams in progressing the carer agenda within the Division.

You will ensure, in consultation with Service Manager for Carers, that up to date information and initiatives are distributed across services and implemented accordingly.

Connect carers to a range of relevant support, including use of community assets, voluntary or community services, and core commissioned services.

Remaining in regular contact with Carers to gain formal and informal feedback on services.

Ensuring Carers are aware of and invited to participate in engagement events and support groups.

Promote carer self-management and independence by ensuring that carers receive the information and support they need to help build resilience, and enable them to manage their caring role.

Provide information on health promotion to carers, and ensure that any health and wellbeing needs are identified and appropriately address through sign posting to relevant services.

To provide or sign post carers to information about mental illness, treatment and other relevant information and resources in the community.

To liaise with other colleagues and organisations for and on behalf of carers and where relevant ensure carers are signposted to an advocacy service.

To liaise regularly with Service Manager for Carers to ensure up to date information is being disseminated and seek clinical supervision with them to ensure professional development

Ensure carers receive appropriate and timely information.To provide emotional and practical support to carers. This will be achieved in various ways, but particularly by providing individual contact with carers and ensuring Carer Leads are doing the same.

Promoting Carer Awareness

Champion carer statutory rights and needs and ensure that any national and local information is cascaded to colleagues and local teams/service areas.

Support initiatives aimed at promoting carer identification such as the

Triangle of Care, Johns Campaign and the development of the Carers passport.

Ensure the role of the Carer Champion is recognised and valued with mental health teams and services.

Ensure the role of Ward or Team Carer Leads are recognised and valued with mental health team and services.

Ensuring Carer Leads are fully implemented within each team and ensuring Carer Leads are appropriately trained, informed and supported to fulfil their roles.

Encouraging an environment and culture whereby all Services are aware of the significance and benefits of fully involving, including and supporting carers.

Completing Carer Awareness training with staff members across all teams to enable staff to feel competent and confident to support carers.

  • Working knowledge of appropriate therapeutic models to engage and retain service users in treatment and to support them achieve their vocational goals.
  • NVQ Level 3 or equivalent additional relevant training or experience relevant to the duties of the post
