Occupational Therapists, Nurses, Social Workers - this is not your usual community mental health team!

We are looking for people full of compassion, enthusiasm and energy to work alongside people in the community to provide high quality care and strong clinical leadership within a countywide community team.

We are an evolving service and our aim to create a co-designed service, where we can work in partnerships with housing, CMHT's & third sector organisations to meet the rehabilitation and recovery needs of individuals accessing our service. We will provide intensive support to people in their own homes with a view to working towards independence.

The service will adopt a psychological approach, to aid understanding of the individual's distress and, have dedicated space for regular case formulation and reflective practice sessions

The service runs over 7 days 8-8.

Based both in the South and the East of the County.

Main duties of the job

The post holder will will support the Team Manager in providing effective frontline management of the multi-disciplinary Community Rehabilitation Mental Health team. In working in partnership with those people accessing the service, they will ensure access to the full range of specialist assessment and interventions.The post holder will work with the CMHT and lead on assessing, planning, and the implementation of evidence based care to those people accessing the Communtiy Rehabilitation Team (CRT). The post holder will work intensively and, at times, creatively, with those accessing the service. The post holder will provide professional advice and support to other agencies and members of the multi-disciplinary team and work closely with partnering organisations in supporting people to integrate into and live well their community. In agreement with the Team Manager, the post holder will provide leadership and supervision to junior members of the team.

To provide a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools.

To work flexibly across a 7-day-a-week service.

To delegate aspects of the patient treatment plan to other members of the team, whilst maintaining overall responsibility, where appropriate, for patients care..

To ensure the clinical caseload and clinical practice of nursing members of the Community Rehabilitation Team is of the highest standard of clinical care.

To manage case-load and service delivery in accordance with and in order to contribute towards the attainment of Service/Trust productivity requirements.

To undertake the Individual Recovery Outcomes Counter - a facilitated self-assessment that uses the HOPE framework to assess and measure personal recovery.To produce and inform care plans, risk assessments and I.ROC reviews.

To formulate care plans collaboratively, informed by the IROC and based on the recovery goal, of the person accessing the service.

Work with individuals to assess their mental wellbeing, recognise and make sense of their experiences, to identify their related needs and circumstances and enable them to understand, manage and where appropriate change their behaviour.

Plan, implement, review and improve interventions to meet peoples identified needs and manage their risk.

To assess carers and families needs and develop, implement and review programmes of support for cares and families.

Protect people from abuse, neglect and harm.

Enable individuals to develop independent living skills and live in the community.

To work collaboratively and promote effective working relationships with members of the multi-disciplinary team, ensuring effective and appropriate clinical decision-making, within the Community Rehabilitation Team and both with internal teams and external agencies.

To maintain accurate and timely clinical records and to co-ordinate and monitor those of the team.

To adhere to codes of professional conduct and ethics, plus associated legislation.

To develop clinical practice having due regard to contemporary evidence base and guidelines

To demonstrate empowering leadership skills within the Community Rehabilitation Team and seek opportunities in the local and national area as to promote and develop the profession.

To participate in Clinical Governance activities, including: induction, supervision, personal development review, health and safety, risk management and audit.

To undertake specific project work or any other duties as negotiated with the Community Rehabilitation Team Manager.
