Job summary

This is a really exciting opportunity to join a developing service. The Here4You Access Team "went live" in January 2022, it was initially a 12 month project to but quickly established itself as a "must have" service.

Despite individual members of the team being based across the entire county of Lincolnshire we have become a very cohesive, supportive team. There is at least one full time position available due to additional funding as well as internal progression of staff.

Main duties of the job

The Here4You Access team practitioners' main duties are; to screen all incoming referrals to Lincolnshire Partnership Foundation Trusts, Children and young peoples emotional wellbeing and mental health services. Provide advice and signposting information in regard to other appropriate services available across the county and nationally. Provide advice to professionals regarding emotional wellbeing and mental health of the CYPs they work with. Liaising with other CYP services in order to ensure a positive journey for CYPs and their families when navigating the complex health and social care system. Providing supervision and training to colleagues and students.

AdministrationProficient use of the clinical system to upload and locate demographic information as well as clinical information where appropriate.Ensuring accurate and appropriate information is recorded within the young persons clinical notes.Robust risk assessing and triage including ensuring all information is accurate, timely and relevant to CYP care.

Qualifications

Essential

  • RMN/ RNLD/Social Worker/Occupational Therapist with current registration.
  • Educated to degree level in mental health/social care or another relevant subject or equivalent experience within the mental health field of working.

Desirable

  • Therapeutic interventions qualification.

Experience

Essential

  • Experience of working with people with mental health needs, including those with emotional wellbeing needs
  • Experience in risk management of people with emotional wellbeing/mental health needs.
  • Good knowledge of universal children's services.

Desirable

  • Experience of working with children and young people who have mental health or emotional wellbeing needs.
  • Experience of delivering therapeutic interventions.
  • Experience of providing support to junior staff.

Skills
