Band 5 / 6 depending on experience.
An excellent opportunity has arisen to come and be part of our dynamic Occupational Therapy service. We are looking for a highly motivated and dedicated Occupational Therapist with post qualification experience or a newly qualified Occupational Therapist who has an interest in working with adults with learning disabilities and complex health needs in the Community. Banding of the role will be based upon clinical experience, skills and knowledge.
You will work as part of a well-established multi-disciplinary team and you must be a compassionate, organised, a team player with excellent communication skills and effective time management skills. You will ideally have experience in clinical assessment, care planning and appropriate clinical decision-making skills. You will also be required to be accountable for your own caseload management and will identify Occupation Therapy interventions which you will joint work with staff within the team to deliver. Your enthusiasm is just as important as your experience as full training can be provided for a successful applicant.
The community hub team offers a range of specialist health interventions to people with a learning disability within their place of normal residence, respite and day care placements. The interventions offered are conducted within the agreed pathways offered by the Learning Disability Service:
- Physical Health Liaison

- Mental Health Liaison
Main duties of the job
Plan, implement, review and improve interventions to meet people's identified needs and manage their inherent risk.
Contribute to case conferences and reviews, supporting service users and carers to engage with services and contribute to their care.
Accept ongoing accountability for service users, hold and manage caseload (complex cases) and supervise and/or mentor junior staff and learners as required in the provision of direct/indirect occupational therapy care.
Safe and effective recommendations and/or prescription and ordering of equipment required by individuals to promote independence and maintain a safe environment.
