Job summary

We have an exciting opportunity to recruit a Band 5 to Band 6 Developmental Role. We are now implementing a locality hub approach to the service so we can enable more localised ways of working, in support of the community transformation model. As Band 5 practitioner you will want to develop your skills to become a band 6 practitioner. A comprehensive and tailored developmental plan will be designed with the post holder depending on training development needs identified. You will be part of a locality team responsible for a designated caseload of patients who have been identified as experiencing their first episode of psychosis within that locality. You will work as part of our integrated multi-disciplinary team, and participate in the clinical decision-making process to determine the most appropriate management plans for all service users who are under the care of the team. Main duties of the job

To provide a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive discharge planning, delegate aspects of the patient treatment plan to other members of the team, whilst maintaining overall responsibility for patients care, Work with individuals to assess their mental health, recognise mental illness and identify their related needs and circumstances; and enable them to understand, manage and where appropriate change their behaviour. Plan, implement, review, and improve interventions to meet people's identified needs and manage their inherent risk.

Assess carers and family's needs and develop, implement, and review programs of support for cares and families. Protect people from abuse, neglect, and harm. Enable individuals to develop independent living skills and live in the community

Work collaboratively and promote effective working relationships with members of the multi-disciplinary team, ensuring effective and appropriate clinical decision-making, both within the EIP Team and with external agencies.

Participate in Clinical Governance activities, including induction, supervision, personal development review, health and safety, risk management and audit.

Develop effective and supportive links with other health and social care staff. Create networks that improve the pathway of care for clients referred to the EIP Team.

Visit people in their own homes throughout your locality.

You will work alongside other members of the MDT including Psychiatrist, Psychology, CAARMS assessors, Mental Health Professionals and Peer workers. Your locality team will cover either the East (Louth Skegness and Boston), West (Gainsborough and Lincoln) or South (Grantham, Sleaford, Stamford and Spalding) of the County and there surrounding areas, and will be based at one of these localities which will be in discussion with your locality manager. Once developed you will transition into the role of Band 6
