Job summary

We are now implementing a locality hub approach to the service so we can enable more localised ways of working, in support of the community transformation model. As an Early Intervention Practitioner, you will be part of a locality team responsible for a designated caseload of patients who have been identified as experiencing their first episode of psychosis within that locality. You will work as part of our integrated multi-disciplinary team, and participate in the clinical decision-making process to determine the most appropriate management plans for all service users who are under the care of the team. We are looking to recruit individuals who are motivated and enthusiastic, with proven abilities to work positively with service user, carers and family. Main duties of the job
The post holder will be responsible for the co-ordination of care needs, assessment, planning, and implementation of evidence-based care to a defined group of service users and their carers within the service catchment area whilst under the guidance of senior staff. The post holder will provide professional advice and support to other agencies and members of the multi-disciplinary team.
