Job summary

An opportunity has become available for a Senior Mental Health Practitioner to provide specialist mental health care as a valued member of the Primary Care Network (PCN). The post will be based within the Spalding PCN and It will include working with primary and secondary care and the neighbourhood teams to locally support the mental health transformation and improve the lives of people who access healthcare services.

Main duties of the job

Key elements of the role will include (but not be limited to):o Providing specialist clinical knowledge and interventions to service users in primary care setting. This will include working with the practice team to support people with highly complex mental health needs/serious mental illness.o Developing and implementing clinical processes that contributes towards the continuous improvement of mental health provision across the primary care network (multiple GP practices)o Providing best practice training, mentorship and guidance to mental health practitioners and other roles within the primary care/ neighbourhood MDTo Managing a caseload of highly complex/SMI presentations working closely with the locality secondary mental health service, to maintain personalised care plans which focus on what matters to the person and avoid escalation to.o To perform the duties of a non-medical prescriber, ensuring that appropriate medication is prescribed, monitored, and administered in line with current best practice and the Trust Medicines Management Policy.The post holder will be encouraged to contribute to service development, and there will be opportunities for CPD and clinical supervision as professional development is actively encouraged.

  • Evidence of specialised continued professional training (degree level) in clinical practice
