Job summary

An opportunity is available for an experienced Mental Health Practitioner to provide specialist mental health care for the newly developed Integrated Placed Based Team.

The post is based in the Spalding locality covering the Primary Care Networks, Munro Medical Centre and Beechfield Medical Centre.

There are two posts available, one will coordinate care for working age adults and one will coordinate care for older adults.

These roles provide great opportunity for an enthusiastic, motivated, and committed practitioner to work integrated within the Lincolnshire health, social and voluntary care system. Main duties of the job

The successful candidate will be an experienced registered practitioner with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative. They will demonstrate adaptability, excellent communication skills and self-motivation. In addition to undertaking timely mental health assessments, you will take on a caseload and provide evidenced based therapeutic interventions that are person centred, ensuring that people feel involved in decision making of their care.

Essential

  • Degree in mental health and social care or equivalent
  • 1st Level Registration (NMC,HCPC)
  • Evidence of specialised continued professional training (degree level) in clinical practice.
  • Evidence of specialist training at diploma level or equivalent
  • Evidence of continuous professional development

Experience

Essential

  • Post-Registration experience of working with adults with mental health needs
  • Sound knowledge of the national agenda for mental health
  • Sound knowledge of clinical/ risk assessment and understanding of confidentiality
  • Sound knowledge of community care issues, local health and social structures, functions and boundaries
  • Sound knowledge of the Mental Health Act
