We have a very exciting opportunity to recruit to a Pschological Intervention Facilitator (PIF) within the Community Rehab Service.

We are looking for people full of compassion, enthusiasm and energy to work alongside people in the community to provide high quality within a countywide community team.

We are an evolving service and our aim to create a co-designed service, where we can work in partnerships with housing, CMHT's & third sector organisations to meet the rehabilitation and recovery needs of individuals accessing our service. We will provide intensive support to people in their own homes with a view to working towards independence.

The service adopts a psychological approach, to aid understanding of the individual's distress and, have dedicated space for regular case formulation and reflective practice sessions

The service runs over 7 days 8-8.

Main duties of the job

The role of a Senior Carer Peer Support Worker (PSW) has been developed specifically for individuals who have lived experience of caring/supporting someone who has/is experiencing mental distress. It is essential that a carer peer support worker within LPFT Mental Health Services has lived experience of caring for someone who has or is accessing mental health services. Applicants who do not indicate they have this lived experience will not be considered.

Under the supervision of a qualified member of staff and with peer support, the role of the carer peer support worker is to provide support to carers whose relative or friend is currently open to the service that the carer PSW is working within. Through appropriately sharing and drawing on their own experiences, the carer peer support worker will inspire hope and belief that recovery is possible, and support families to feel hopeful about the future. This peer support role may include developing and co-facilitating carer groups, including peer support groups and Recovery College courses (likely to be out of 9-5 hours).

To act as a connector and work collaboratively to establish and develop links with the wider peer support network both internal and external to LPFT.

Act as a role model, showing a professional and caring attitude towards families, and other professionals and colleagues.

Where appropriate, to provide guidance and practical support to carers to better enable them to support their relative or friends plan of care/treatment.

Recognising the emotional impact of caring for somebody experiencing significant distress, provide emotional support and listening ear for carers.

Provide lived experience perspective through participating in carer therapy groups, helping carers believe they are not alone and that change is possible

As a core member of the multi-disciplinary team, the Senior Carer Peer Support Worker will take a lead role in working alongside the other Peer Support Workers and students on a group basis and provide a supportive and leadership role within the Recovery College. They will also have the opportunity to co-work with other colleagues.

Guiding/supporting carers through periods of transition (i.e. when their relative is transitioning out of, or across, services)

In line with the Triangle of Care initiative, support enhanced communication and collaboration between carers and mental health professionals, and ensure that carers have all the right information to support the recovery of their relative.

Support carers to identify access support within their local community and, where appropriate, link into carer organisations.

Develop and coordinate opportunities for carer participation and co-production within LPFT carer services. For example specific carer courses within LPFTs Recovery College.

Liaise with representatives from universal services, as appropriate, and facilitate and model good communication between services.

Comply with and carry out safe practice in accordance with Trust policies and procedures, and local and national policy and legislation.

To support induction, supervision and work-related training for Peer Support Workers.

Maintain strict confidentiality of patient information at all times and ensure accurate record-keeping in accordance with Trust Policy.

Facilitate access to community groups and networks that enable service users and /or carers to participate in community activities, in order to maximise opportunities for taking on socially valued roles, positive identify and support people moving on from clinical services.

To work flexibly across the week, this may include some weekend and evening work.

To work with expert peers and expert professionals, applying the principles of co-production by developing a learning environment that promotes equality and mutuality.

To act as a connector and work collaboratively to establish and develop links with the wider peer support network both internal and external to LPFT.

Model personal responsibility, self-awareness, self-belief, self-advocacy and hopefulness via the telling of own recovery story to inspire and instill confidence in patients and staff alike.
