Job summary

Would you like to be at the forefront of exciting new developments within Adult Learning Disability services as a Trainee Advanced Clinical Practitioner. Do you have a passion to work within specialist earning disability services? Do you want to contribute your experience skills to new ways of working across an integrated care system (ICS)? We are seeking to appoint 3 highly motivated individuals who would relish the opportunity to practice in the Adult Learning Disability Service within LPFT as a Trainee Advanced Clinical Practitioner.

LPFT's adult learning disability service, provides specialist health support to people with learning disabilities who require assessment and/or treatment for their physical and mental health, including support with behaviours of concern. The adult learning disability service operates from three multi-disciplinary hubs, across Lincolnshire, which delivers a range of specialist provision as well as a county-wide community home assessment and treatment team.

Applications are welcome from healthcare professionals who have current registration and whom meet the current requirements to become non medical prescribers. The post is a band 7 for the period of the ACP training, upon qualification as an ACP the post will be a band 8a. Main duties of the job

The post holder will work together with the Consultant Psychiatrists for the service to support the behaviour of concern and mental health pathways, with a commitment to the STOMP/STAMP pledges. You will therefore ideally have experience of working with patients who have a learning disability. The post holder will have an advanced level of autonomy, providing clinical leadership at all levels and will work in partnership with patients, their carer's and internal/external colleagues within the MDT to deliver high quality, evidence based health management. ACPs in LPFT are not purely clinical however: research and education activities are part of your job plan. Through these endeavours, the post holder will support the development of, and drive improvements in, the care for patients with a learning disability and colleagues' skillsets within the service and relevant sections of the Lincs ICB. We are actively involved with Universities across the midlands, supporting clinical training and research.We are committed to staff development and the post holder will receive regular management and clinical supervision along with relevant training to meet recommended NICE guidelines. We have an active ACP journal club and our strategic vision for ACP places us as a front runner in this field amongst mental health trusts.

Job responsibilities

Whilst a Trainee Advanced Clinical Practitioner, the trainee will develop the skills and knowledge required to undertake the role as outlined below.

The ACP role is a national role, transferable across organisations. Therefore, the role will follow that national format which includes the 4 pillars of practice.

Clinical Practice

Develop advanced clinical knowledge to independently assess, diagnose, plan, implement and evaluate treatments and interventions with patients presenting to the service with complex undifferentiated or undefined presentations.

Participate in/ observe physical and mental health examination as indicated by the presenting individuals condition; enabling the development of skills to autonomously analyse complex clinical signs and investigation results, considering differential diagnoses and diagnose conditions, initiating treatment as required.

Participate in / observe clinical procedures appropriate to the history and physical examination of the presenting individual including further invasive testing and treatments requiring highly developed skills and precision.

Participate in/ observe in requesting of investigations such as blood, urine and other laboratory tests, electrocardiographs (ECGs), ultrasound scans, X-Rays and computed tomography (CT) scans if deemed appropriate to role in accordance with IR(ME)R regulations and as local policy allows.

Develop the skills required to analyse multiple sources of data including patient / service user history, physical examination and investigation findings when making diagnoses, clinical judgements and evaluating care provided; presenting this information, to speciality /senior medical staff for advice when scope of practice is exceeded

Learn to autonomously formulate appropriate management plans, formulates clinical decisions and often complex treatment plans to manage acute mental or physical illness and acute episodes of chronic illness including at referral, admission or discharge.

Participate in/ observe the continuous revaluation of findings and clinical response to treatment and establish an appropriate ongoing management plan accordingly.

Learn to prioritises health problems and intervenes appropriately in complex, urgent and emergency situations, including leading/participating in emergency life support procedures.

Participate in/ observe the management of clinical events involving patients, often requiring unpredictable and high levels of mental and/or physical effort according to the dependency and clinical need.

Observe the provision of clinical cover to clinical wards and teams according to patient and staffing needs.

Develop the skills to prescribe and review medication (as an independent prescriber) for therapeutic effectiveness appropriate to patient need and in accordance with best/evidence-based practice and national and local protocols and within the individuals scope of practice and legal frameworks.

Learn to Integrate both pharmacological and non-pharmacological treatment in patient care/management plans including psychosocial interventions where required

Participate / observe in the assessment of patients with mental health needs using local policy and guidelines and refers to the appropriate services.

Acts as a developing resource for staff, advising on local, national and Trust policy, procedures and guidelines ensuring patient safety and clinical governance.

Management

Work in partnership with the existing senior medical, nursing and AHP teams in driving departmental development and change.

Learn how to manage conflicting views and liaises between groups where there may be conflict.

Highlight and address areas of witnessed poor practice and manage the situation appropriately.

Support staff development to maximise potential, encouraging everyone to learn from each other and from external good practice.

Support patients, service users and carers and staff during difficult situations arising in the clinical area e.g. breaking bad news or following an unexpected event

Produce accurate and complete documentation and patient records consistent with legislation, policies and procedures.

Possesses excellent time management and personal organisation skills.

Act as a developing knowledge resource in how to initiate and manage the impact of change.
