Main duties of the job

This post is essential for the delivery of a modern, specialist mental health service across all LPFT older adult teams/services. The post holder will use his/her experience, skills and initiative to develop, co-ordinate and provide a high-quality clinical pharmacy service to patients and their carers within Lincolnshire Partnership NHS Foundation Trust's (LPFT) older people and frailty (OP&F) division.

The postholder will be an expert in psychiatric therapeutics and provide high quality expert clinical pharmacy advice in all areas of older adult and dementia services across LPFT and the Integrated Care System (ICS) where applicable. They will produce and implement good practise guidelines, procedures, protocols and pharmaceutical care pathways to improve medicines optimisation into and out of the services within the OP&F division.

They will be a core member of multidisciplinary team meetings and oversee and undertake clinical audit and quality improvement initiatives where required to optimise and develop services.

They will work closely with other managers and leaders inside and outside of the Trust to ensure clinical pharmacy service delivery to the OP&F division are provided by a competent and versatile pharmacy workforce.

The postholder will be a role model to and provide clinical supervision and line management to junior and trainee pharmacy staff.
