Job summary

As a senior member of the Pharmacy Department, the post-holder will:o Lead on the delivery and development of high-quality clinical pharmacy services in line with medicines optimisation initiatives consistent with local and national objectives.o Support the medicines optimisation agenda at individual clinical service level. This includes the provision of clinical service level medicines usage reports and to contribute to policy and protocol development to support the safe and cost-effective use of medicines.o Monitor adherence to medication related legislation, governance, and the safe and economic use of medicines through means of clinical research and audits.o Manage a team of pharmacy staff and lead on the provision of an efficient medicines management training programme for Trust staff.o Work alongside the Formulary Pharmacy team to provide support to the Drugs, Therapeutics and Medicines Management Committee (DTMMC) and to support the operational processes for entry of new medicines to the Trust formulary.o To support the Deputy Chief Pharmacist in actioning and managing patient and medication safety alerts and incidents, review and complete regular medication related incident reports, and support the services with implementation of any action plans as necessary.o Deputise for and support the Deputy Chief Pharmacist in delivering the medicines safety agenda and in the provision of clinical pharmacy services Trust wide.

Main duties of the job

The post holder will be responsible for:o Ensuring conformance with the legal and professional requirements cross the clinical services dispensaries.o Lead on the development and delivery of the medicine's optimisation agenda at individual clinical service level.o Lead on the review and development of relevant trust policies, procedures, guidelines related to the safe, effective and efficient use of medicines.o Support and work with the Deputy Chief Pharmacist to develop Trust wide medication incidents and medicines management and quality and safety reports as required by the Trust.o Support and oversee the work of Clinical Pharmacists working in the section to ensure delivery of an effective, safe, and efficient pharmacy service.o Reducing risks associated with medicines use by leading on departmental initiatives and contributing to the Trust agenda for medication safety.o To lead on the audit agenda for the department and actively contribute to quality improvement initiatives.o To lead on the development of Key Performance Indicators (KPIs) for clinical and operational services.o Assist with the development and delivery of relevant medicines related teaching to pharmacy and other trust staff.To act as a line manager to specialist and rotational pharmacists and be a role model to all pharmacy staff as a senior member of the team.
