Specialist OT/ Physiotherapist - Integrated Discharge Team

Job summary

We have an excellent opportunity for a Physiotherapist or Occupational Therapist to join our established Integrated Discharge Team based at DRI.

The successful candidate will be dual skilled in both Physiotherapy and Occupational Therapy competencies working to drive patient flow.

As part of the Team you will play a vital role in providing quality care for our patients over 7-days, whilst delivering the Trust's goals of reducing length of stay, admission avoidance and reducing avoidable readmissions. This post will also support the Trust's Frailty Assessment Unit, Acute Medical Unit, Gresley Unit comprising of Kestrel, Kingfisher and Mallard wards along with the discharge to assess pathway.

Main duties of the job

You will provide specialist assessment, diagnosis and treatment of patients with complex problems as an autonomous practitioner within the Integrated Discharge Team

You will support the Team Lead in the operational management of the Integrated Discharge Team and service developments on a local or service-wide level

You must have excellent communication and interpersonal skills, with the ability to motivate not only patients but staff. You must be organised, flexible and able to empower others.

You will provide Therapy assessment and treatment to patients who attend our Emergency Department, Frailty Assessment Unit and Acute Medical Unit to avoid unnecessary hospital admissions and support timely discharge of patients, accounting for their therapy and social care needs. You will link with community health and care providers to meet the needs of patients.
