Job summary
The post of Hepatology Pathway Navigator (HPN) is a new role in South Yorkshire and Bassetlaw and as such, will develop and evolve in response to the needs of our liver services. The primary focus will be supporting SYB Clinical teams who hold responsibility for the identification, assessment and surveillance of people with a high risk of developing liver cancer including those with advanced fibrosis and cirrhosis.
Main duties of the job
To plan and organise own workload and priorities to ensure fulfilment of all defined roles and responsibilities. Support the development of the SYB Hepatology Pathway Navigator role, working with clinical leads and operational colleagues to underpin practice with clear Standard Operating Procedures and with a robust governance framework.
Using defined digital systems, the postholder will be responsible for maintaining the registry of liver surveillance patients to support their clinical pathway, ensuring patients are invited for surveillance tests and clinical reviews in accordance with protocol.
To work closely and communicate regularly with key clinical teams across SYB who are involved in the care of patients requiring liver surveillance: working as a visible member of those respective teams.
Ensuring data completeness and validation against key indicators and data items to enable reliable data utilisation and analysis - feeding both local and national reporting metrics.
Working closely with both Trusts and the Cancer Alliance BI Team, under the supervision of clinical and operational colleagues, regularly review pertinent data, recognising its relevance to quality and performance and its integral link with service transformation. The postholder should be capable of reviewing and understanding source data and able to extrapolate themes, trends and data quality issues
