Job summary
To provide a comprehensive, specialised nursing service to patients in relation to colorectal conditions, stoma care and fistula management.
Acting as a key member of the multidisciplinary team, advisor and expert resource to other health professionals within the hospital and community Trusts.
Main duties of the job
Provide expert advice and support to patients and carers from diagnosis and throughout the patient pathway.
Provide clear, accurate and appropriate written and verbal information according to the patients needs.
Act as a point of contact, as a coordinator of care, liaising with MDT and others
Attend MDT and other relevant meetings, contributing specialist nursing knowledge and skills and acting as a patient advocate.
Assess, plan and implement care for individual patients.
Responsible for assessing, negotiating and making optimal site for stoma formation in patients undergoing elective colorectal surgery
Educate patients, relatives about basic principles of stoma care.
Provide ongoing support
