Job summary
This is a secondment post for 5 months, and is an exciting new role within Doncaster and Bassetlaw Teaching Hospitals as part of the ongoing development of the Hepatology service. This role will involve exiting networking opportunities throughout health care sectors in order to develop the integration of services. This post holder will be motivated and dedicated team member in developing, and improving the care for those at risk of developing liver disease, and those with liver disease.
Main duties of the job
The post holder/CNS will provide liver health assessment clinics and the delivery of diagnostic Fibroscanning across the network of Doncaster and Bassetlaw including referrals from GP surgeries, weight management service and drug and alcohol treatment providers and IAPT services.
The post holder will provide advice, information and support for participants referred in for liver health check/Fibroscan such as directing to the British Liver Trust, and local service provisions.
This may include those who have a diagnosis of liver cirrhosis/advanced Fibrosis and enter the HCC surveillance pathway.
The post holder will be responsible for care delivery for patients enrolled for Fibroscan assessment, and those individuals recalled for repeat Fibroscan assessment of their liver disease/fatty liver.
The post holder will work with the lead liver nurse in working as a part of a multidisciplinary Team working across the sectors, which may require onward referral to support services to reduce and support the patient with their liver disease; or to reduce the risk of liver disease developing.
