Macmillan Living with and Beyond Cancer/Personalised Care Lead

Job summary

The Macmillan Living With And Beyond Cancer Programme is working to ensure that people diagnosed with cancer are living as healthy and active a life as possible during and after treatment. More people than ever before are living with and beyond a cancer diagnosis. The impact of cancer does not suddenly stop when the treatment is over.

Main duties of the job

Project manager deliverables

1. NHSE Personalised care and support deliverables 2022-24

2. Support the implement SYB Cancer Alliance PSFU pathways at DBTH

3. Cancer Care Review transfer of information from DBTH to primary care to inform CCR quality linking with Living Well Lead Manager and new Macmillan bid LWMC

4. Linking DBTH teams to services in community support services via the POD and Doncaster (Living Well) & Bassetlaw (Aurora)

5. Support the delivery of PC&S workstream activity of the Cancer Alliance in Doncaster

6. Engage with the a peer group of project managers and Lead Cancer Nurses involved in the implementation of PC&S

7. Actions from Previous Project Manager handover report

8. Maximise take up and quality of PCSP through the use of COSD data reports, educational offers and quality improvement tools.
