Job summary

We are advertising a post with the aim of developing newly established services at our trust. The post will be spread across the division of Acute Medicine and General Medicine, with the intention to contribute to the recently established Infectious Diseases/General Medicine ward team. The post will include on-call duties in acute medicine.

The purpose of this post is to provide a clinical service within the Divisions of Medicine and Emergency & Acute Medicine to patients of the Doncaster and Bassetlaw Teaching Hospitals NHS FT (DBTH).

DBTH has demonstrated a commitment to develop the care of patients with infection problems with the appointment of two specialists in Infectious Diseases & General Medicine since 2022. The trust now delivers inpatient and outpatient Infectious Diseases services for patients living within the catchment area of the Doncaster, Bassetlaw and Montagu Hospitals.

Main duties of the job

The post holder will be expected to:

  • Provide an acute medicine service to the patients of DBTH, with anticipation that acute medicine work will be undertaken at both Doncaster Royal Infirmary and Bassetlaw Hospital
  • Provide a general medicine service, supporting the specialist Infectious Diseases inpatient team at Doncaster Royal Infirmary.
  • Provide consultation and advisory service to clinical colleagues in the Trust and Primary Care.
  • Work within multi-disciplinary teams in providing a high-quality clinical care, with participation in cross-site and virtual ward meetings.
  • Collaborate with colleagues to deliver a high quality, timely service and to work towards meeting the Trust's and departmental objectives.
  • Participate in the Acute Medicine on-call rota to ensure that both emergency and urgent services are provided out of normal hours, to be shared equally with Consultant colleagues in post. The Acute Medicine rota does not include overnight cover however there is a late shift until 9pm every week and 1:8 weekends with 10-hour shifts. Whilst undertaking on-call duties attend the hospital site to review patients when clinically necessary. The consultant on-call must be prepared to attend willingly whenever requested to do so by a member of the medical and nursing team. Whilst undertaking weekend on-call duties, attend the hospital to undertake patient reviews.

The infectious diseases/general medicine ward has been established in 2022. The successful candidate will work alongside two consultant ID/GIM colleagues, with the support of an ID/GIM specialist trainee and junior doctors who will work on the ward. The post holder will be expected to deliver 2-3 ward rounds each week. In addition to the ward, the post holder will work with the Infection specialists to deliver the Trust sepsis review. The successful candidate will be expected to provide cross cover for the GIM duties of the ID/GIM consultant colleagues for periods of leave.

We have a vision of further developments within the Infection department that complements the Acute Medicine service to support the establishment of an OPAT service utilising existing Virtual Ward services. The post holder would have the opportunity to support and develop this service, utilising existing collaborative services between the Acute Medicine and Infection departments.

The post holder will:

  • Participate in general medicine ward rounds and MDTs.
  • Forge links between acute medicine and department of infection.
  • Develop working policies with relevant hospital services and departments, local General Practitioners, nursing teams and the wider multi-disciplinary teams.
  • Develop innovative practices in clinical patient management, focusing on high quality care and emphasis on links between emergency and acute medicine, general medicine and department of infection.
  • To share with colleagues the responsibility for teaching and training of trainees, junior medical staff, nursing staff, primary care colleagues and medical students.
  • To have responsibility for ensuring active participation in continuing professional development (CPD).

Education and Development:

  • There is a wide teaching role within the post, to train medical students, nurses and junior doctors, as they rotate through the department.
  • It is expected that the post-holder will be a clinical/educational supervisor to some of the training doctors on rotation, as well as taking part in their regular weekly teaching timetable. 1hr/week SPA time is afforded for this role.
  • There is a monthly all staff teaching session which the post holder will be expected to attend and contribute to. This includes a regular audit meeting, where again, consultant contribution is expected.
  • The study leave allowance is 30 days/3 years pro-rata. It is expected that these will be used, in addition to SPA time, for CPD towards revalidation as per the recommendations of the Royal College and the various specialty bodies. The trust is fully committed to supporting and assuring revalidation, by way of a dedicated support team and IT infrastructure package.
  • The trust has a programme of mandatory training which all consultants are expected to undertake.
  • For appointees new to the consultant role, or who may have had a career break, the department strongly encourages mentorship support

Clinical Governance:

  • Attendance at the monthly specialty clinical governance group is expected.
  • Further contribution to clinical governance activities in the form of local, regional and national audit participation, guideline writing/updating, and undertaking other relevant projects would also be expected.

Management and Leadership:

  • There will be an expectation to take on management roles, such as Specialty or Foundation training co-ordinator, education lead or clinical governance lead. For any of these roles, additional SPA time would be granted.

Research:

The Trust has an active Research and Development Department, and provides access to research nurse support, participation in research is encouraged. This may take the form of personal local projects, or collaboration in regional or national projects, such as the current PrEP IMPACT study.

  • You must be on the specialist register for Acute Medicine or General Medicine or within 6 months of being admitted to the register for trainees, if currently in a training programme in the UK. In accordance with the regulations, all other categories of Doctors must be eligible to be on the GMC specialist register (evidence required prior to commencement) to be considered for a consultant appointment by the Advisory Appointments Committee
