Job summary

The post holder will be responsible for supporting with the delivery of education and promotion sessions regarding the virtual ward, to ward staff on a one to one basis and within a group environment. They will liaise with the ward staff and complete the ward discharge check list ensuing the virtual ward criteria is met. They will liaise with patients and family members in regard to Virtual Ward.

Main duties of the job

They will liaise with the ward staff and complete the ward discharge check list ensuing the virtual ward criteria is met. They will liaise with patients and family members in regard to Virtual Ward.

The post holder will also be responsible in the absence of the Ward Clerk for providing an effective and well organised administration/reception service to the Virtual Ward based at Doncaster Royal Infirmary and Bassetlaw General Hospital. The post holder will assist in the uploading/ discharging of patients onto virtual ward; and for filing of admission/ discharge paperwork, and data tracking.

Job responsibilities

  • Uploading patient details onto Virtual Ward
  • Liaising with patients and families regarding the role of the virtual ward and the support that will be provided.
  • Ensuring the discharge process is completed on CaMIS, email and paper documentation is filled appropriately.
  • Liaise with Community partners regarding the patient information / contact detail.
  • Supporting with reporting to the MDT on patients on the Virtual ward
  • Answering the telephone promptly, taking messages/action and re-directing calls as necessary.
  • Attend all mandatory training in accordance with Trust
  • Participate in the Trusts Annual Appraisal process, ensuring that all agreed objectives are completed.
  • To identify any shortfalls in personal ability and develop action plans to overcome these in conjunction with the Ward Manager.
  • Undertaking any other duties which may be allocated specifically from time to time by the ward manager/ ACP, band 6 or band 7.
  • Ensure that the delivery of all clerical details complies with departmental and Trust policies, procedures, and guidelines.
  • Ensure that any communications by self are perceived in a constructive and helpful manner.
  • Ensure all relevant policies are adhered to at all times.
  • To be conversant in the use of electronic communication systems, personal computer systems, normal office equipment.

Qualifications

Essential

  • Functional skills Level 2 English Language and Maths Qualification or GCSE Grade C or above in Maths and English Language
  • Level 2 NVQ or equivalent experience, or willingness to work towards relevant qualification.

  • Good computer skills and ability to navigate IT systems proficiently
  • High level of accuracy and attention to detail
  • Express a sympathetic and diplomatic nature, especially when dealing with illness, bereavement and distress
