Job summary
This is an exciting opportunity to become a specialist Band 6 Speech and Language Therapist working in the A rated Stroke pathway at Doncaster and Bassetlaw Teaching Hospitals NHS Trust.
The successful applicant will ensure service delivery to patients transferred to the Fred and Ann Green Rehabilitation centre, a purpose built inpatient unit based in Montagu Hospital located very close to the M18 and A1.
There is an opportunity to work across the stroke pathway providing specialist SLT input on the Hyperacute Stroke Unit at DRI as well as supporting the developing Neurorehabilitation service at both DRI and Bassetlaw.
Working alongside the current Band 6 SLT and supported by Band 7 working across the stroke pathway, the successful applicant would help to drive service improvements and quality initiatives.
In addition to working with the dedicated stroke MDT at Montagu hospital the successful applicant will be welcomed into the award winning and inclusive SLT team at DBTH being part of regular professional meeting's and having the opportunity to be involved in VF clinics and the developing FEES service.
Part time hours, job sharing and flexible working will be considered for the right applicant.

Main duties of the job
Applicants should have specialist experience of working with dysphagia and have a post basic qualification in dysphagia. A flexible and pro-active approach is essential for this post, as are excellent communication and team working skills.
There is full senior support and a structured supervision process in place.
The post holder will support the Therapy Team Lead and existing Senior Speech and Language Therapists with the operational management of Speech and Language Therapy team. They will contribute to the education, training and development of Band 5 Speech and Language Therapists, Therapy Assistants, Multidisciplinary staff and students. The role will include undertaking professional development appraisals and service evaluation including audit.

Job responsibilities
- Manage and prioritise the speech and language therapy caseload on the Rehabilitation Units at Montagu Hospital in accordance with departmental guidelines.
- To independently assess and manage a range of communication and swallowing difficulties.
- To be able to define a differential diagnosis after analysing the evidence from assessments and provide appropriate management, including the use of AAC devices.
- To be competent in post basic dysphagia assessment and management.
- To provide holistic assessment and management of every patient referred, and to be able to differentially diagnose and provide timely informed sound clinical judgements, which will include quality of life and ethical decisions.
- To co-ordinate day to day management of in-patient and outpatient caseloads and to prioritise urgency, including complex dysphagia and communication needs.
- To liaise with senior therapists across the acute and stroke care pathway and into the community teams to ensure continuity of patient care
- To refer into and attend videofluoroscopy/nasendoscopy clinics if/when appropriate to do so.
- To use a high level of skill and experience in contributing to ethical and quality of life decisions within multidisciplinary teams
- To continually monitor, evaluate and modify treatment plans to meet the patients changing needs.
- To recognise own professional boundaries and be able to seek professional support from senior clinicians and expert practitioners.
- To regularly liaise with professionals from other agencies.
- To contribute to interagency teams by discussing own and other professionals input around clients needs ensuring a well-coordinated care plan.
- To have a flexible approach to service delivery maintaining quality standards across a variety of settings and provide cover across hospital sites for dysphagia patients during period of annual leave, sickness, absence etc.
- To be able to manage emotional consequences of working with distressing conditions.
- To be alert to hazardous situations and act accordingly, and potential risks to patients, staff and self and take appropriate action.
- To discharge patients when the duty of care has been fulfilled and notify relevant agencies.
- To attend multidisciplinary team meetings.
- To maintain up to date and accurate case notes in line with professional standards and local Trust policies.
