You will be required to cover the 7 day rota which will include; days, evenings, weekends and bank holidays on a rotational basis.
An exciting opportunity has arisen for the role of Patient Flow Co-ordinator in Emergency Services Care group.
The purpose of the Flow Co-ordinators role is to ensure that patients have a seamless journey and discharge and that the patients pathway is effectively managed across site.
We are looking for a motivated and enthusiastic individual to join our dynamic team to contribute to high quality patient experience. You will play a pivotal role in supporting the Ward managers and Matrons to support patient pathways from admission to discharge/ transfer.

Main duties of the job
The Flow coordinator is responsible for liaising with Emergency Department, Clinical Site and other Flow Co-ordinators and receiving areas to ensure effective communication about capacity is maintained and appropriate early escalation is facilitated.
It is desirable that the applicant has recent experience in a health environment. The applicant must be computer literate have GCSE English or equivalent. Good communication skills are essential and ability to work with a multi-professional team. The applicant must be able to work flexibly and prioritise workload effectively, and use own initiative in certain work related issues.
The successful applicant will be expected to work across both sites as required, Monday to Sunday working hours TBC.

MAIN DUTIES/RESPONSIBILITIES
Specific Daily duties of a Flow coordinator:
- Ensure Electronic capacity data is recorded as near to real time as possible by prompting key person responsible for patient flow for the care group.
- Monitor activity for admissions and escalate timely and appropriately to Ward Manager/Matron when trigger points are hit.
- Be vigilant in the assessment of flow and escalate timely any blockages to the to Ward Manager/Matron.
- Ensure actions and owners of actions identified at Operational flow meetings are supported in completion in specified times.
- Support to Ward Manager/Matron by providing accurate and up to date information on capacity and flow across site and escalate issues in a timely manner to ensure effective patient flow is maintained across site.
- Ensure that all other relevant DBH information systems are updated by the relevant members of the MDT accordingly.
- Coordinate repatriation from external organisations and field to Ward Manager/Matron.
- Work in A&E department ensuring all breach targets are met and flow is maintained, escalating any delays to NIC, EPIC & Matron
- Additional Duties
- Support with booking ambulance, and other, transport, in compliance with Trust guidelines, for patients discharge or transfer in agreement with patients and relatives with appropriate use of the Discharge Lounge.
- Liaise with pharmacists and pharmacy technicians for TTO drug provision.
- Support MDT to ensure that social care forms i.e.
- Integrated discharge team referrals
- RAPTS
- Community Matron
- District Nurse
- Acute Oncology Service/Peaks Alert
- CRT
- Ensure TTO and discharge letters are available and completed by relevant member of the MDT for all discharges in a timely manner by working closely with Discharge Co-ordinators.
- Ensure any patient investigations that are outstanding are obtained and communicated. Identify delays in patients journey or discharge and escalate to the appropriate to Ward Manager/Matron.
- Liaise with MDT team to ensure referrals are completed where necessary.
- Keep patients and relatives fully informed. Liaise with patients, relatives and MDT to confirm discharge details and destination and ensure appropriate documentation is completed including patient surveys.
