Job summary

Doncaster & Bassetlaw Teaching Hospitals NHS Foundation Trust are offering opportunity for a 12 month, fixed term Band 6 Speech and Language Therapy role.

This is a unique opportunity for a Speech and Language Therapist with experience in Stroke and Neurology to take on the role of Senior Speech and Language Therapist at Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust (DBTH).

The successful applicant will ensure service delivery to inpatients referred for Speech and Language Therapy on the stroke pathway. This will involve working flexibly across the stroke pathway including the Hyperacute stroke unit at Doncaster and the Stroke Rehabilitation services at the Bassetlaw and Montagu sites. There may also be non-stroke cover required within the role.

The exact configuration of these sessions will be dependent upon service requirements. This role will also encompass HASU acute weekend working in the new year, when staffing ratios allow. This should be approximately one weekend in every five.

The successful post holder will be crucial in meeting targets for timely assessment and rehabilitation and ensuring timely discharge to community services.

Main duties of the job

Applicants should have specialist experience of working with dysphagia and have a post basic qualification in dysphagia. A flexible and pro-active approach is essential for this post, as are excellent communication and team working skills.

This post is part time and will contribute to 7 day service provision. There is full senior support and a structured supervision process in place.

The post holder will support the Therapy Team Lead and existing Senior Speech and Language Therapists with the operational management of Speech and Language Therapy team. They will contribute to the education, training and development of Band 5 Speech and Language Therapists, Therapy Assistants, Multidisciplinary staff and students. The role will include undertaking professional development appraisals and service evaluation including audit.

The post would ideally suit someone who would like the opportunity to:

  • Develop/continue with their operational and management skills
  • Develop/continue with their skills in acute stroke as well as stroke rehabilitation
  • Develop/continue with their acute non-stroke skills
  • Provide assessment and therapy to stroke patients with complex communication difficulties, including facilitation of capacity assessments
  • Work as a member of the stroke pathway multi-disciplinary teams, sharing and contributing to team meetings and updating others of own work.
  • Work as a member of the wider DBTH SLT service.

Job description

Job responsibilities

  1. Manage and prioritise the speech and language therapy caseload on the Rehabilitation Units at Montagu Hospital in accordance with departmental guidelines.
  2. To independently assess and manage a range of communication and swallowing difficulties.
  3. To be able to define a differential diagnosis after analysing the evidence from assessments and provide appropriate management, including the use of AAC devices.
  4. To be competent in post basic dysphagia assessment and management.
  5. To provide holistic assessment and management of every patient referred, and to be able to differentially diagnose and provide timely informed sound clinical judgements, which will include quality of life and ethical decisions.
  6. To co-ordinate day to day management of in-patient and outpatient caseloads and to prioritise urgency, including complex dysphagia and communication needs.
  7. To liaise with senior therapists across the acute and stroke care pathway and into the community teams to ensure continuity of patient care
  8. To refer into and attend videofluoroscopy/nasendoscopy clinics if/when appropriate to do so.
  9. To use a high level of skill and experience in contributing to ethical and quality of life decisions within multidisciplinary teams.
  10. To provide specialist treatment programmes to communication patients including co-ordinating, managing and providing the LSVT service for Parkinsons Disease patients.
  11. To continually monitor, evaluate and modify treatment plans to meet the patients changing needs.
  12. To recognise own professional boundaries and be able to seek professional support from senior clinicians and expert practitioners.
  13. To regularly liaise with professionals from other agencies.
  14. To contribute to interagency teams by discussing own and other professionals input around clients needs ensuring a well-coordinated care plan.
  15. To have a flexible approach to service delivery maintaining quality standards across a variety of settings and provide cover across hospital sites for dysphagia patients during period of annual leave, sickness, absence etc.
  16. To be able to manage emotional consequences of working with distressing conditions.
  17. To be alert to hazardous situations and act accordingly, and potential risks to patients, staff and self and take appropriate action.
  18. To discharge patients when the duty of care has been fulfilled and notify relevant agencies.
  19. To attend multidisciplinary team meetings.
  20. To maintain up to date and accurate case notes in line with professional standards and local Trust policies.
  21. To support patients and staff in the assessment of Mental Capacity with reference to a patients care and future care.
