Job summary

An exciting opportunity has arisen for a full time fixed term, Band 6 Nurse Specialist to join the Highly Specialist NHS England funded Hyperinsulinism team. This post will be for 12 months to cover maternity leave.

The post holder will work within a CNS team who have a nursing responsibility for complex patients with Hyperinsulinism and their families, working in close partnership with the Consultants and the whole multidisciplinary team.

The post-holder will also be part of the General Endocrine CNS team, with occasional support offered to the diabetes team.

Applicants need to possess some experience in the care of patients with Hyperinsulinism as well as excellent communication and being able to work in a small team providing appropriate support and a smooth patient journey.

The successful applicant will be expected to uphold and demonstrate the trust values at all times, as well as ensuring staff around you do the same.

Main duties of the job

o Provide information, clinical advice and support to children and families with hypoglycaemia and act as a resource to other professionals involved in that child's care.

o Be a competent, knowledgeable and visible practitioner within the specialty ensuring the provision of high quality evidence based nursing care for the children and their families.

o Ensure that each child is assessed, and their care planned, implemented and evaluated in negotiation with the family and other staff caring for the child, and that this is accurately communicated and documented.

o Liaise with clinical, non-clinical and facilities staff in order to maintain a safe, friendly and welcoming environment for the children, families, visitors and staff.

o Act as an advocate for the child and family ensuring the provision of appropriate information and support services.

o Ensure that each child within the caseload has a named lead clinician and designated care co-ordinator to co-ordinate their care and ensure a smooth ongoing journey through effective discharge planning.

o Ensure effective communication between all members of the multidisciplinary team both internally and externally to the Trust, other hospital departments, relatives and visitors.

o Develop and maintain the clinical skills and knowledge necessary to provide clinically effective, evidence based patient care.
