This post will be fundamental in enhancing our future Healthcare Transition Pathway.

We are looking for a highly motivated and passionate individual who has existing experience of a youth worker role with ideas to support and champion the voice of our young people, within our unit and wider hospital community. This support and education will help enable and empower our young people, to take responsibility for their kidney disease and its management. The role will see you working alongside the wider multidisciplinary teams both in the paediatric and adult setting.
