Job summary
The critical care departments at Great Ormond Street Hospital (GOSH), London, UK, invite applications for a training fellowship in intensive care medicinefor children, which will start on the Neonatal Intensive care unit (NICU). Rotations can include time on the Neonatal Intensive Care Unit (NICU), Paediatric Intensive Care Unit (PICU), Cardiac Intensive Care Unit (CICU), and the Children's Acute Transport Service (CATS).
We are able offer bespoke paediatric and neonatal critical care placements for an anaesthetist, paediatrician, cardiologist, paediatric emergency physician or other relevant specialty wishing to gain paediatric intensive care medicine (PICM) experience. This could be a 6-24-months NICU/PICU/CICU/CATS placement, or alternatively, a two-year fellowship (including PICU, CICU and CATS) for those wishing to train as paediatric intensivists. Experienced PICM applicants approaching consultant level may wish to spend time on the 'PICU senior tier' - a pre-consultant leadership role with the opportunity to lead ward rounds and run the shop floor.
In addition, we offer time on the NICU/PICU/CICU within this application as stand-alone 6-12 -months posts. We welcome applications from registrars from a range of backgrounds with a range of experience and can discuss individual training needs.

Main duties and responsibilities
1. ICU weekly duties and timetableAs a tertiary/quaternary referral hospital many of the patients seen here are extremely complex and encompass all specialities. The consultant intensivists are fully committed to intensive care with no other clinical responsibilities, so all care is consultant led.The department provides facilities for complex monitoring and treatment procedures, including different techniques for mechanical ventilatory support, intracranial pressure monitoring and continuous veno-venous haemofiltration. Advanced support includes ECMO/VAD which is carried out in the CICU. There is separate consultant cover for PICU, NICU, CICU and CATS with two consultant led ward rounds on each unit every day. Formalised unit teaching activities are arranged around weekly teaching days and one monthly teaching day involving fellows from other North London PICUs. Bedside and ward-based teaching is provided within each unit. There is a hospital Grand Round at lunchtime on Wednesday.The appointees will be required to take a share in all activities of the unit. This will include partaking in "on call" duties, which are an integral part of the full shift as outlined. Although an agreed duty rota is carefully followed, occasional emergency cover may be required. Unit and hospital protocols exist, and the post-holder is expected to adopt management which takes note of these.
