Job summary
This is a unique post which will sit within the Transforming Care Team that is innovative and energetic in its engagement with the Transforming Care Agenda. The Transforming Care Team is a community based service that works collaboratively with IST (Intensive Support Team) and CTPLD (Community Team for People with Learning Disability) (and CMHT/PCART/CAMHS/ALMHS/NASS and Children's services - across all service providers for people with LD/ASD) in implementing the Transforming Care Agenda, that is all about improving health and care services so that people with a learning disability and/or Autism can live in the community with the right support. Central to the role of the Admission Avoidance and Discharge Facilitator support, is supporting the crisis management function and role by being able to respond to crisis in the community with proactive, innovative and targeted responses to meet a person's needs. This will involve and include the patient, family, care providers, professionals and anybody else involved in meeting the person's needs.
Main duties of the job
The other function of importance is to collaboratively work with the discharge in mind of all patients within the cohort who are at risk of admission, in hospital or long stay facilities across all service providers and LD or mental health and Forensic Inpatient Services. The role will include chairing CTR's (Care and Treatment Reviews). The role involves collaborative working with the local CCG, providers, other CCGs and Social Care and is a prerequisite that is central to supporting people with the transforming care cohort.

Knowledge & Experience
Essential
- Educated to relevant degree level or professional qualification e.g. nursing , social work, allied healthcare
- Knowledge and experience of working in a Learning Disability service/provider
- Knowledge and experience in transforming Care
- Be an NMP or be willing to work towards the NMP qualification as an essential for the role
Desirable
- Knowledge of working in acute or community mental health units and mental health pathways and care planning
- Learning Disability Nurse qualification
- Experience of training/presenting to others / Experience of coaching/mentoring others
- Willingness to train towards the NMP qualification
- Experience of supporting services and commissioners to develop a life-long pathway of support and crisis avoidance
