Job summary

Our Single Point of Access is an essential service which supports patients and professionals to make referrals into the NHFT community Nursing services and community based palliative care. This is an exciting time for SPoA as we are engaged in a transformation plan which will develop streamlined electronic referral for routine referrals.

The post holder will support the Single Point of Access team (SPoA) to deliver high quality referral management and administrative processes to facilitate a smooth patient journey to care with our Community Nursing and Intermediate Care Team services. Included in this are referrals for unwell adults, some of which are subject to a rapid (2 hr) response, palliative care and medication referrals as well as more routine referrals for assessments by our specialist teams. The post holder will lead a team who provide the 'front door' of community nursing services and ensure that referrals are managed promptly and urgent needs are escalated quickly to the appropriate teams.

  • line management of SPoA staff
  • support operational leaders in transformation and quality improvement
  • use evidence/best practice to facilitate and create an environment which supports safe effective management of routine and urgent referrals within the scope of the Community Nursing/ICT specifications
