Job summary

The role of the Reconnect worker will identify those in need of health services on release, assist them to plan for these and link them into community Reconnect services.

Northamptonshire NHS Foundation Trust are pleased to announce a new project delivering the a RECONNECT service within HMP Bedford. Reconnect is a newly commissioned service and offers the opportunity to be a part of developing this new national project.

It is recognized that people in prison are at greater risk of poorer health than the general population. Proactive encouragement and support to engage with health services can prevent avoidable differences in health outcomes and positively influence the risk of reoffending. Leaving prison is a time of change; the Reconnect service aims to ensure that the transfer to community health services is effective and comprehensive, and that people are supported to access all the services that they need after release.

Main duties of the job

The role of the Reconnect worker will identify those in need of health services on release, assist them to plan for these and link them into community Reconnect services.

By starting to work with vulnerable individuals before they leave you will have a positive impact on health inequalities and encourage individuals to take responsibility for their own health needs. The service has been initially commissioned for a period of 24 months .

You will

  • Have experience working in health care and/or the criminal justice system
  • Be able to work independently and part of the wider healthcare team
  • Have excellent communication skills and apply these to build relationships with vulnerable people
