Job summary

We are currently looking for motivated and enthusiastic Band 6 Nurse, Social Worker or Occupational Therapist to join the Urgent Care & Assessment Team (UCAT) Home Treatment Team, formally (CRHTT) based at Campbell House, Northampton. The team is a 7 day a week, 365 day a year service providing urgent response assessments and home treatment as an alternative to admission for people experiencing mental health crisis.

The multi-professional team aims to work with individuals and their carers to manage a mental health crisis by identifying, assessing and care planning for service users as well as averting a crisis wherever possible through prevention and early intervention.

We are looking for:

- Experienced clinicians with excellent assessment, dynamic risk assessment/management and communication skills.

- Proactive individuals with a passion for a recovery based approach, promoting social inclusion and enhancing an individual's wellbeing.

- Dynamic and enthusiastic clinicians with the resilience to work in a demanding and diverse environment.

Main duties of the job

The post holder will be responsible to undertake very urgent front line assessments from GPs and other referrers, making clinical decisions for appropriate care planning and/or onward referrals. In addition to creating robust home treatment plans;providing direct patient care to people in their home who are acutely mentally ill.

To take a lead role in the the formulation, development, implementation and review of individual treatment plans.
