Supported and working closely with a dedicated TCA service manager, you will be overseeing three dedicated teams whose agenda is hospital avoidance, quality care delivery and managing behaviours that challenge. This is a unique permanent post which will sit at the pinnacle of the Transforming Care Agenda (TCA) and is energetic in its engagement with the Transforming Care Agenda. The Transforming Care Team (TC), Intensive Support Team (IST), Community Swift Response team (CSRT) and Community Team for People with Learning Disability (CTPLD) are community-based teams that thrive to offer outstanding care. Your role will be to operationally manage TC, IST and CSRT teams and together work across all service providers for people with LD/ASD in implementing the Transforming Care Agenda, which is all about improving health and care services so that people with a learning disability and/or Autism can live in the community with the right support. You will be part of a thriving team working closely with the service manager(s), Head of service and Assistant Director.
Main duties of the job
The Community Swift Response Team is a new small multi-disciplinary team that has been developed in line with the Transforming Care agenda which is a National driver for the Learning Disability and Autism Services. The Team compromises Nursing, Occupational Therapy, Speech and Language Therapists and Senior Support Workers. The Community Swift Response Team will provide a specialist wrap around provision and intervention for adults, children, providers and families requiring an urgent response who are unable/do not have the skills to support an individual whose presentation has escalated away from baseline resulting in the placement being at risk and/or risk to quality of life.
The Intensive Support Team (IST) is a community-based service delivering intensive multi-disciplinary healthcare assessment and treatment for individuals fourteen years and over who have learning disabilities that are experiencing mental ill health and/or presenting behaviour that challenges their existing systems of support. The service provided is person centred and tailored to the individual and their environment, considering their ethnic and cultural backgrounds. The team provides direct intense support, joint working with other services (e.g., Community Team for Adults with Learning Disabilities; Community Mental Health Teams; Community Teams for Children, Disabled Children's' Team, Transitions Services) and/or through offering consultancy to other professionals involved.
About us
As Operational Manager, you will work as part of a dynamic team in delivering the Transforming Care agenda in Northamptonshire. In your role, your span of control will include the direct management of clinical leads and senior allied health and/or nurses in the 3 aforementioned teams who together with admin workers will work with you to ensure the smooth running of these teams that together are geared at admission avoidance and management of behaviours that challenge. You will report to and be directly supported by the dedicated TCP service manager and be part of a team player-based model. You will also closely work with CTPLD and 1 Willow Close (respite care service for clients with profound learning disability) operational managers as part of a team that believes in collective learning and team development.
