Job summary

The Community Swift Response Team is a multi-disciplinary team that has been developed in line with the Transforming Care agenda which is a National driver for the Learning Disability and Autism Services. Supported and reporting to the service and operational manager and as part of the Community Swift response Team, the post holder will ensure the nursing provision for both children and adults is integrated and cohesive within the countywide community Learning Disability and Autism Service, to provide a seamless pathway for individuals who have a learning disability and or Autism spectrum condition who are presenting with a mental illness and / or severe challenging behaviour.

The Team compromises of Nursing, Occupational Therapy, Speech and Language Therapists and Senior Support Workers. The Community Swift Response Team will provide a specialist wrap around provision and intervention for adults, children, providers, and families requiring an urgent response who are unable/ do not have the skills to support an individual whose presentation has escalated away from baseline resulting the placement being at risk and /or risk to quality of life.

The service provided is person centered and tailored to the individual and their environment, using a needs or strengths-based intervention before / during crisis to reduce the need for hospital admission. considering their ethnic, diversity and cultural backgrounds

Main duties of the job

The team will work to prevent an escalation through targeted resourcing of the MDT, supporting crisis and reducing the need for hospital admission. This will be provided through direct input either in the individual's home and or the crisis house working alongside the current care provider. The team will provide direct "hands on care" and therapeutic engagement which could include the following: role modelling, implementation of structure, sensory strategies, communication systems, boundaries and intervention derived from assessment by the CTPLD/ IST/Mental Health teams through the challenging behaviour and recovery care pathway and considering the Core Capabilities Framework for Supporting people with Autism.

The Community Swift Response Team will work in close partnership with the individual and family / carers, Transforming Care team, IST, CTPLD, Mental health teams and other services, such as Social Services, generic Health Services, independent and voluntary services, Advocacy Services and Commissioners.

The post holder will work as part of the Community Swift Response Team within a multi-disciplinary framework.Individualswill have a diagnosis of a learning disability and or Autism who are presenting with a mental illness and / or severe challenging behaviour; whose presentation has moved away from baseline and/ or crisis.

The post holder will be able to work with clients with the full range of mental health diagnoses who find themselves in a crisis and requiring specialist short term support as an alternative to hospital. You will work very closely with the relevant teams UCAT / IST depending on diagnosis, who will provide clinical direction and support. You will be proactive in seeking out the positive support networks available in the community for everyone which may help them in their recovery.

The post holder will work with the MDT providing clinical leadership within their role and proactively participate in and provide clinical and management supervision and appraisal, providing mentorship and support to junior members of staff, students, and trainees.

Co-ordinating the individuals care and applying evidence-based practice the Post holder will working partnership within the key stakeholders, IST, wider Learning Disability Service, mental health, and other agencies, to move away from the crisis point and focus on desired outcomes. Ensuring a seamless pathway for clients.

The post holder will work flexibly to respond to emerging needs and effectively address the needs of the clients. The team operates a 24hoursservice. Ourwill be required to work shifts between the hours of 08.00 to 20.00 hours, 7 days a week, 365 days a year, with flexible working outside of these hours when required.

Knowledge and Experience

Essential

  • Recognised Nursing qualification RNLD/RMN, educated to Diploma Level
  • NMC Registration
  • Health & safety awareness, including Clinical risk assessment and risk management.
  • Post registration experience working with people with Autism and/or a learning disability who have challenging and complex needs (MH/ CB)

Desirable

  • Knowledge of current issues and best practice in Autism and Learning Disability Nursing with an awareness of National policies and drivers within these areas including Valuing People, Transforming Care, Mental Health legislation.
