Band 6 Community mental health practitioners, Countywide Early intervention service (N-Step) community Mental Health Team (full time 37.5 hours Monday to Friday).

Main duties of the job
You will be working with individuals and their families from aged 14 years upwards CAMHS experience will be welcomed.
The post holder will be working in the county localities as a Specialist Community mental health practitioner, you will be at the forefront of shaping the team's service model, working alongside your MDT colleagues to ensure that services provided meet the National access and waiting standards for Early intervention services. This will include cognitive behavioral therapy and Brief family interventions. Also ensuring the services are responsive, flexible, and innovative. You will be involved in evidence-based assessments when requested, and in developing and delivering recovery focused interventions and community groups.
You will maintain Care-Coordinator responsibilities for a small case-load of service users with first episode Psychosis, and participate in day to day team duties.
The successful candidate must have excellent communication and assessment skills, be confident in their ability to support service users and their families with Psychosis, and most importantly be a whole team player.

Clinical
- Conduct specialist mental health assessments and evidence based interventions for a defined caseload presenting with first or early episode of psychosis, irrespective of diagnosis. This approach will incorporate the following elements; systematic psychological assessment, psycho-education, family interventions, medication management, motivational interviewing approaches, relapse prevention planning, assessment and treatment of co morbid conditions, outcome
- Adopt a recovery based approach that promotes social inclusion, encourages educational, vocational and leisure opportunities for service users and implement interventions accordingly.
- Work alongside clients to collaboratively assist them in meeting their physical health care needs and provide high quality information with regards to the links between physical health and mental
- Work autonomously, utilising the Intensive Case Management approach, adopting the role of Care Co-ordinator for all clients on his/her caseload and applying all aspects of care co ordination including risk assessment and risk management, in accordance with the local care programme approach framework and within the context of the multi disciplinary
- Carry continuing responsibility for the assessment of care needs, planning, implementation and evaluation of individualised care for all clients on his/her caseload, ensuring that care is client centred and driven by the needs and preferences of the client, family and friends.
