Job summary
This is a rare and exciting opportunity to join our Salisbury Hospice and Specialist Palliative Care Community CNS Team on a substantive post working full time hours.
We are seeking to recruit a highly motivated nurse ideally with clinical palliative care knowledge and experience at Band 6, whom is confident in decision-making, innovative and with a passion to always improve.
The role will involve providing expert symptom control advice and psychological support. You will hold a caseload of patients in a geographical area, linked to GP practices. You will based at Salisbury Hospice and travel into the community setting, so access to transport is essential.
We have strong links with all of our external professional colleagues and are regularly involved with GSF and MDT meetings.
Main duties of the job
The Specialist Palliative Care Nurse is an expert in ensuring high quality palliative care is delivered to patients and their families. They are able to work independently, as well as part of the Salisbury Hospice and Specialist Palliative Care Service and with external health care teams.
To offer the patient and families the skills of a clinical nurse specialist in symptom control, social, emotional, psychological and spiritual care.
To offer advice, support and education to the primary health care teams, community and hospital staff where appropriate, as part of a multi-professional team.
To work from using a multi-professional approach to care.
Experience of holding conversations around advance care planning and using end of life care tools in a community environment
To participate in quality initiatives within the department.
Experience of delivering both formal and informal teaching.
