The Trust is investing in our Early Supportive Discharge team to enable them to support patients home as soon as medically fit with additional care provided by the Council. The service works to a'discharge to assess' model, in partnership with Wiltshire Council's reablement team to provide therapy in the patient's own home and ensure patients are given the support they need.
The team is based at the hospital, facilitating discharge plans and liaising with other services before taking patients home. ESD team complete assessments in patients own homes to ensure they have the adequate care and therapy they need to support their return to independence.You will be expected to effectively communicate with patients, families and the wider team, support senior staff members in planning and coordinating discharges. The role also requires you to take part in the organisation and admin required alongside supporting with the collection of data to evidence the service outcomes. You will also be involved with providing care and practicing exercise plans in patient's home.
The ESD team is a 7 day service from 08:00-18:00 therefore flexibility in working pattern is required. A standard working week is 08:00-16.00. Having a full driving license and access to a car is essential.
Main duties of the job
The team will work both within the acute hospital on Wards and in the community, to enable a smooth transition and consistency of care through the discharge process. You will be required to work closely with the multidisciplinary team, the patient and families along side the registered therapist to plan and deliver discharge home as soon as able when medically fit.
Patients may require a discharge home visit and/or follow-up visits in the community to assess them in their own environment. The main focus is always to assess functional ability in a familiar environment where possible, to support people to regain their optimal level of functioning, and to achieve what matters to them.
You will be expected to support patients with activities of daily living in their home, including personal care and meal provision, supporting an ethos of home is best.
This post will particularly suit a motivated, organised and flexible individual who ideally has rehabilitation or caring experience, however this is not essential.
