We are seeking a dedicated Clinical Nurse Leader to champion high standards of palliative and anticipatory care within our community services. This role is ideal for someone who is enthusiastic about educating and empowering the workforce to deliver exceptional care at the end of life.
End of life and palliative care is a key priority within the NHS, and we are committed to getting it right. We are looking for a nurse who brings:
- A wealth of experience in community nursing
- Strong leadership and communication skills
- A collaborative approach to working within multidisciplinary teams
- A deep understanding of the complexities of palliative care
You will work closely with district nursing and community therapy teams to ensure patients receive holistic, compassionate, and coordinated care.

Main duties of the job
- Clinically this role involves providing anticipatory care for those with palliative conditions and additional support in community settings for palliative patients with psycho-social complexity through autonomous clinical practice.
- To be ultimately accountable for managing and planning care for patients with complex palliative and end of life needs in the community, escalating needs in a timely manner.
- To work with primary care and partner agencies to ensure palliative patients are identified, anticipatory care is delivered and onward care is secured to reduce the need for secondary care intervention.
- Play an active part in service improvements for Palliative and End of Life Care and management of people with complex needs in the Trust.
- This includes contributing to Trust wide palliative and end of life pathway development, monitoring pathway implementation and auditing patient outcomes in relation to End of Life Care initiatives.
