Intensive Home Treatment Support Worker

Job summary
The support worker will be a part of an exciting new Intensive Home Treatment Team (IHTT) with Buckinghamshire CAMHS in concordance with new NHSE guidance. This team will be focused on supporting individuals who present with complex needs and who require intensive risk support within the community.
The post-holder will be an integral part of a multi-disciplinary team who can offer support in developing formulations with the aim to provide care to young people and families in a holistic way.
The post holder will be working alongside other clinicians in the team in holding a shared caseload of young people who require active support and/or treatment.
The aim of the Intensive Home Treatment Team is to support and engage children, young people and families with complex needs by means of individualised home and community treatment packages.
Main duties of the job
Under the supervision of Intensive Home Treatment (IHT) staff within the Bucks CAMHS Complex & Vulnerable Young People Pathway, you will be supported to:
- Deliver clinical care and manage a caseload, including developing and evaluating care plans with qualified staff and supporting individual recovery needs.
- Maintain accurate and timely clinical records, ensuring confidentiality and contributing to risk assessments and safeguarding procedures.
- Support service users in emergency situations, provide handovers, and implement recommended strategies acrossprofessional networks.
- Attend and contribute to various meetings, including clinical reviews, care planning, and service development discussions.
- Liaise with leadership and undertake additional duties as required to support service provision and team operations
