We are looking for a dynamic, creative, enthusiastic clinician to take on a role of Team Manager to lead the Primary Care Mental Health Hubs. You will be responsible for overseeing the Primary Care Mental Health practitioners working across various PCNs in Buckinghamshire managing the daily operations of the hub; ensuring the delivery of high-quality mental health services, fostering a supportive & collaborative environment for both staff and service users.

As the Team Manager you will need to have effective leadership, excellent interpersonal and communication skills to work across the system. You should be enthusiastic about improving the patient journey, experience and care. For this role you will need to be able to work flexibly with our services in Mental Health, Primary Care, the Voluntary Sector and Social Care.

Main duties of the job

  • Provide clear, visible leadership and strategic vision to ensure a safe, efficient, and evidence-based user experience.
  • Manage and support Mental Health Practitioners (MHPs) in Primary Care Networks within the Bucks locality.
  • Lead and support the delivery of an integrated Mental Health Primary Care pathway for Adults and Older Adults with Serious Mental Illness (SMI).
  • Ensure delivery of a high-quality, innovative, integrated service that breaks down barriers to care and supports healthy living in communities.
  • Offer line management and support to staff working within the Hubs.

The ability to travel independently between sites within the Trust is essential for this role.
